I wonder if the reduction applies before even using relics to things like enlarged heart being that the capacity starts off halved.

Wait so does "Efficacy" mean it increases the effectiveness of healing and life steal, or does this thing literally provide 20% life steal on everything?

The ring is in the boss room even if you chose the alternate ending and didn't kill any boss.

When you defeat the boss, you are left in the boss room. You must leave the map and then return back to the map and the ring will be on the ground where the boss fight took place.
it is not necessary to do the alternate ending which requires a random annex zone, simply killing it, leaving the zone then returning is enough
