AnonymousFound in the same bell tower as the Blooming Heart relic. To get Slayer's Crest simply take elevator to the top of the tower. In my run discovered it in The Nameless Nest.

there is an invisible wall between the steps leading up to the slayers crest setpiece where you can input the code

On the slayer's crest setpiece, there are 4 runes that appear if you let the elevator go up on its own. I don't know what it's supposed to mean :/ but someone oughta remember this
