When doing the invincible engineer build DON'T USE THIS! Use the Indignant Fetish.

Then you can still have a healing item. It is impossible to actually get 100% damage reduction.

100% damage reduction status does not equal no damage at all. Not sure how damage reduction is actually calculated. Still taking damage a t160% reduction.

Ok, some more detailed information I got. Not sure if it only spawns in Cotton's Kiln or Butcher's Quarter, but I found mine in Cotton's Kiln anyway.
You will find a burning wooden wall with a breakable entrance that has an explosive barrel around it. If you cannot find any place too obvious, then look for a background, any background with higher resolution and more details, typically something will hide there.
Once you are in, there will be piles of wood and skeleton, and there will be crows with blue glow, the same as what huntress attack. You will find the Cleansing Stone lying at the end of the place, and a wooden box in a broken wooden house around. The wooden box is a quest item that you can interact middle of it to open, inside will be the Effigy Pendant.

Okay, I found the passage. For me it was about a quarter of the way through the area. There was a short stairway off to the left of the main path that led to an area that was roughly "T" shaped. At the top-left of the "T" there was a red exploding barrel next to a wooden panel covering the secret passage.

Look for a breakable pallet hiding an opening. Crouch and go through the hole. Go forward, should run into a bunch of glitchy birds (you'll know them when you see them) and some adds.

There is a broken wall to the left with a hole you can duck into with a wooden box. Inspecting the box in your inventory gives you the effigy pendant.

The Cleansing Stone amulet was really close by too. Good hunting all!

Gee what nice location description. "Players need to access a secret passage to find it." Bruh...whats the point of like half the items here on the wiki if I just need to look it up somewhere else to know how to get it.
